Q. How can I get a copy of my child's immunization record?

A. Contact our office and a copy of the immunization record will be prepared for you. You may pick the record up or we can mail it to you.

 

Q. Why do I need to sign a release?

A. Medical records are protected and need your signature before they can be released except for the release of treatment, payment, and health care operations.

 

Q. How do I get my medical records transferred?

A. You can sign a release at your previous Dr.'s office or you can sign a release at our office.

 

Q. How long does it take to get my records?

A. It will take approximately two weeks to get the records together and ready to send.

 

Q. Can I get a copy of my medical records for myself?

A. Yes, you will still need to sign a release and on the form you should place your name and address in the "send to" area.

 

Q. How long is a referral to a local specialist good for?

A. Referrals to local specialists are good for 90 days. If a patient needs the referral updated they will need to call their primary care physician for the update.
