BILLING QUESTIONS

Billing is handled through the Sedona Centralized Business Office. Payments can be mailed to 602 35th Avenue, Moline, IL 61265, and they can be reached at 877-854-3551 or 309-736-4011 with billing questions.

For your protection from identity theft, we ask for photo IDs of all our patients.

New Patient

When you call our office, please be prepared to provide the following information:

Your full name, date of birth, current address and phone number. For female patients, it will be helpful to provide us with your maiden name in case we need to locate any previous records for you. We will also ask you what type of insurance you have, who is the primary cardholder and the ID number on the card. If you have a preferred name that is different from your given name and would like us to address you as the preferred name, please let us know.  Our computer system allows us to indicate your preferred name and we will do our best to address you as you wish.

Our offices use electronic medical records (charts), so we will ask for your location preference for:

  • Office Location
  • Laboratory
  • Pharmacy
  • We will also ask you to choose a primary care physician

When you arrive at our office, please be prepared to present your insurance card for our billing office.  

Please carry your insurance card with you at all times to your visits. You may be asked to provide it for copying at each visit.  Please do not feel offended if we ask to copy your card at multiple visits, we have your best interest in mind, so that you will not receive inappropriate bills from an insurance plan that is no longer active or has changed its benefit. The laboratory and x-ray department may also ask for copies of your insurance cards since they will bill your insurance separately.

After arriving at our office, we will have you fill out some paperwork that will help the physicians and nursing staff with your care.  Please be prepared to list all of your medications and strength, both prescription and over the counter. Please also alert the nursing staff if you have any allergies.

If you have been injured on the job, we will have another form for you to fill out – this will help us assure that the billing office files the claim with the appropriate party or insurance. You will be responsible for filing all other liability claims and auto accidents.

What can you expect from us?

  • Non-urgent visits are usually available in 5 days or less.
  • Urgent illness or injuries are triaged and treated accordingly.  If necessary, your physician may recommend that you be treated in an emergency room or urgent care facility. 
  • Some insurance plans have separate coverage for behavioral/mental or substance abuse health issues. If your insurance offers that benefit, please contact the phone number on your card to be directed for care.
  • Your family physician can determine if you need to be treated or referred to a specialist if necessary.

FAQ

Q. How can I get a copy of my child's immunization record?
Contact our office and a copy of the immunization record will be prepared for you. You may pick the record up or we can mail it to you.

Q. Why do I need to sign a release?
Medical records are protected and need your signature before they can be released except for the release of treatment, payment, and health care operations.

Q. How do I get my medical records transferred?
You can sign a release at your previous Dr.'s office or you can sign a release at our office.

Q. How long does it take to get my records?
It will take approximately two weeks to get the records together and ready to send.

Q. Can I get a copy of my medical records for myself?
Yes, you will still need to sign a release and on the form you should place your name and address in the "send to" area.

Q. Is there a cost for copies of medical records?
We will print a summary of your records at no charge. There is a fee to reproduce part or all of your entire record.
