When you arrive at our office, please be prepared to present your insurance card for our billing office. Please carry your insurance card with you at all times to your visits. You may be asked to provide it for copying at each visit. Please do not feel offended if we ask to copy your card at multiple visits, we have your best interest in mind, so that you will not receive inappropriate bills from an insurance plan that is no longer active or has changed its benefit. The laboratory and x-ray department may also ask for copies of your insurance cards since they will bill your insurance separately. After arriving at our office, we will have you fill out some paperwork that will help the physicians and nursing staff with your care. Please be prepared to list all of your medications and strength, both prescription and over the counter. Please also alert the nursing staff if you have any allergies. If you have been injured on the job, we will have another form for you to fill out – this will help us assure that the billing office files the claim with the appropriate party or insurance.
